To register your child by mail, please print the registration form (one per child), complete it and mail it, along with payment, to the address listed on the form. If you wish to register using the paper form and pay by credit card, please complete the Credit Card Authorization Form attached to the Registration Form and submit both to the address listed on the Registration Form.
